Summary
Non-acetylated salicylates can have some NSAID-like effects on renal prostaglandins, which may reduce the antihypertensive effect of telmisartan and raise the risk of renal impairment, especially with volume depletion from the thiazide. The effect is less pronounced than with conventional NSAIDs, and high salicylate doses matter more than low ones.
What this interaction means in plain words
Hydrochlorothiazide-telmisartan and Salicylates can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of renal prostaglandin synthesis reduces renal blood flow and sodium excretion, which blunts ARB and diuretic effects and strains renal function.
What to do
Monitor blood pressure, renal function and potassium if regular high-dose salicylates are used, and ensure adequate hydration.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
