Summary
Osmotic bowel preparations cause fluid loss and electrolyte shifts, and with a thiazide and an ARB the risk of dehydration, hypotension, acute kidney injury and electrolyte-related arrhythmias or seizures increases. Hydrochlorothiazide adds to sodium, potassium and magnesium depletion, and telmisartan reduces renal perfusion under volume depletion.
What this interaction means in plain words
Hydrochlorothiazide-telmisartan and Sodium sulfate / ?magnesium sulfate / potassium chloride can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive volume depletion and electrolyte disturbance from bowel cleansing, diuretic action and impaired renal autoregulation with ARB.
What to do
Consider holding the diuretic-ARB combination on the day of bowel preparation and the day of the procedure. Ensure adequate fluid intake and check electrolytes and renal function in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
