Summary
Bowel preparation causes fluid and electrolyte losses, and thiazide diuretics add to potassium and sodium depletion and volume loss. Together they can cause dehydration, hypokalemia, hyponatremia and reduced renal perfusion, which is a concern when an angiotensin receptor blocker is also taken.
What this interaction means in plain words
Hydrochlorothiazide-telmisartan and Sodium sulfate / potassium chloride / magnesium sulfate / polyethylene glycol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive fluid and electrolyte loss from osmotic catharsis and thiazide-induced renal potassium and sodium wasting, with volume depletion increasing renal risk alongside the ARB.
What to do
Ensure adequate hydration and consider withholding the diuretic and telmisartan on the day of bowel preparation. Check electrolytes and renal function in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
