Summary
Sulfate-based bowel preparations cause fluid and electrolyte loss, and when taken with a thiazide and an ARB the risk of dehydration, hypotension, acute kidney injury and electrolyte-related arrhythmias or seizures increases. Volume depletion makes renal function dependent on angiotensin II, which telmisartan blocks.
What this interaction means in plain words
Hydrochlorothiazide-telmisartan and Sodium sulfate / potassium sulfate / magnesium sulfate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive volume depletion and electrolyte disturbance from bowel cleansing, diuretic action and impaired renal autoregulation with ARB.
What to do
Consider withholding the diuretic-ARB combination during bowel preparation, ensure good hydration, and check electrolytes and creatinine in high-risk patients.
