Summary
Terbutaline promotes potassium shift into cells and hydrochlorothiazide increases urinary potassium loss, so together they can cause clinically significant hypokalemia. Telmisartan partly offsets this by retaining potassium. Hypokalemia may provoke arrhythmias, especially with high-dose or systemic terbutaline.
What this interaction means in plain words
Hydrochlorothiazide-telmisartan and Terbutaline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-telmisartan belongs to Cardiovascular; Terbutaline belongs to Respiratory.
Why it happens
Beta-2 agonist-driven intracellular potassium shift plus thiazide-induced renal potassium wasting.
What to do
Monitor serum potassium, especially with high-dose nebulised or parenteral terbutaline, and replace potassium as needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
