Summary
Xipamide is a thiazide-like diuretic, so adding it to a regimen that already contains hydrochlorothiazide and telmisartan produces duplicate diuretic therapy and a stronger blood pressure lowering effect. This can lead to volume depletion, hypotension, hyponatremia, hypokalemia and worsening renal function, especially in older or volume-depleted patients.
What this interaction means in plain words
Hydrochlorothiazide-telmisartan and Xipamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive pharmacodynamic effect from two thiazide-type diuretics acting on the distal tubule, combined with the antihypertensive action of telmisartan.
What to do
Avoid routine combination of two thiazide-type diuretics; if used deliberately, monitor blood pressure, serum sodium, potassium and creatinine within the first weeks. Reduce or stop one diuretic if hypotension or electrolyte disturbance develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
