Summary
This combination duplicates valsartan, giving two sources of angiotensin receptor blockade with added hypotension, hyperkalemia and renal dysfunction risk. Hydrochlorothiazide may be used with sacubitril/valsartan, but the hydrochlorothiazide-valsartan product should not be given alongside it.
What this interaction means in plain words
Hydrochlorothiazide-valsartan and Sacubitril / valsartan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Therapeutic duplication of angiotensin II receptor blockade, with added neprilysin inhibition increasing hypotensive and hyperkalemic effects.
What to do
Avoid the combined product with sacubitril/valsartan; if a thiazide is needed, use hydrochlorothiazide alone and monitor blood pressure, potassium and creatinine.
What to ask your pharmacist
