Summary
Salsalate, a salicylate with NSAID-like properties, can blunt the blood pressure lowering of valsartan and hydrochlorothiazide. In combination with a diuretic and an angiotensin receptor blocker it can also reduce renal function, especially in older or volume-depleted patients, and may raise potassium.
What this interaction means in plain words
Hydrochlorothiazide-valsartan and Salsalate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of vasodilatory renal prostaglandin synthesis reduces renal perfusion, sodium excretion and antihypertensive effect, while the combined hemodynamic effects add to the risk of acute kidney injury.
What to do
Monitor blood pressure, serum creatinine and potassium after starting or changing salsalate, and ensure adequate hydration. Use the lowest effective salsalate dose for the shortest time, or choose an alternative analgesic where renal risk is high.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
