Summary
Sulfate-salt bowel cleansing preparations cause fluid shifts and electrolyte abnormalities. With a thiazide and an angiotensin receptor blocker the risk of dehydration, arrhythmia, seizures from hyponatremia and kidney injury is increased.
What this interaction means in plain words
Hydrochlorothiazide-valsartan and Sodium sulfate / potassium sulfate / magnesium sulfate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive volume depletion and electrolyte disturbance from the osmotic purgative, the diuretic, and the renal hemodynamic effects of the angiotensin receptor blocker.
What to do
Encourage adequate hydration and consider holding the diuretic and valsartan on the preparation day. Check electrolytes and renal function in older patients or those with kidney disease.
