Summary
Xipamide is a thiazide-like diuretic, so combining it with hydrochlorothiazide duplicates therapy. This increases the risk of excessive diuresis, hypotension, hyponatremia, hypokalemia and renal impairment, with valsartan adding to hypotension.
What this interaction means in plain words
Hydrochlorothiazide-valsartan and Xipamide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Duplicate inhibition of distal tubular sodium chloride reabsorption leading to additive natriuresis and volume depletion.
What to do
Avoid combining two thiazide-type diuretics; use only one agent. If sequential nephron blockade is intended, choose a different diuretic class and monitor electrolytes and renal function.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
