Summary
Ibuprofen can reduce the antihypertensive effect of olmesartan and, together, they can impair renal function, especially in elderly, volume-depleted or renally impaired patients. Hyperkalemia may also occur.
What this interaction means in plain words
Ibuprofen and Olmesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ibuprofen belongs to Musculoskeletal; Olmesartan belongs to Cardiovascular.
Why it happens
NSAIDs constrict the afferent arteriole by blocking prostaglandins while ARBs dilate the efferent arteriole, reducing glomerular filtration pressure, and NSAIDs also cause sodium retention.
What to do
Ensure adequate hydration and monitor serum creatinine, potassium and blood pressure, particularly at the start of therapy. Avoid regular NSAID use in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
