Summary
Tenofovir is cleared by glomerular filtration and tubular secretion and can itself cause kidney injury. NSAIDs reduce renal prostaglandin-mediated perfusion, and reports describe acute kidney injury when NSAIDs are used with tenofovir, particularly at high or repeated doses.
What this interaction means in plain words
Ibuprofen and Tenofovir disoproxil can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive nephrotoxicity, with NSAID-induced reduction in renal perfusion that may raise tenofovir exposure.
What to do
Use the lowest effective NSAID dose for the shortest time, or choose paracetamol where suitable. Monitor serum creatinine and phosphate, especially in those with renal risk factors.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
