Summary

Ifosfamide is a prodrug that is activated mainly by CYP3A4 and CYP2B6. The ritonavir-boosted regimen strongly inhibits CYP3A4, which may reduce formation of active alkylating metabolites and shift metabolism away from the activation pathway. The clinical consequence is uncertain, but it could include reduced antitumour activity.

What this interaction means in plain words

Ifosfamide and Ombitasvir / paritaprevir / ritonavir and dasabuvir can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Strong CYP3A4 inhibition by ritonavir and paritaprevir alters ifosfamide bioactivation and may reduce exposure to active metabolites.

What to do

Where possible, avoid strong CYP3A4 inhibitors during ifosfamide courses or time the antiviral regimen away from chemotherapy. If they must be combined, monitor for reduced response and for unexpected toxicity.
