Summary

Tricyclic antidepressants can reduce the antihypertensive effect of centrally acting alpha-2 or imidazoline agonists, and moxonidine labeling advises against concomitant use with tricyclics. Both can also cause sedation, and imipramine may blunt blood pressure control.

What this interaction means in plain words

Imipramine and Moxonidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism at central adrenergic and imidazoline receptors, plus additive sedation.

What to do

Monitor blood pressure closely and consider an alternative antihypertensive if control is lost. Watch for excess sedation and orthostatic hypotension.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
