Summary

Tricyclic antidepressants can potentiate the cardiovascular effects of beta2 agonists such as olodaterol, including tachycardia, palpitations and QT prolongation. This may raise the risk of ventricular arrhythmias, particularly in patients with underlying heart disease or electrolyte disturbances.

What this interaction means in plain words

Imipramine and Olodaterol inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive cardiac effects: TCA-related QT prolongation and noradrenergic effects combined with beta2-agonist-induced tachycardia and hypokalemia.

What to do

Monitor heart rate, ECG (QTc) and potassium in patients with cardiac risk factors. Consider an alternative bronchodilator if QTc prolongation or arrhythmia develops.
