Tricyclic antidepressants can potentiate the cardiovascular effects of the beta-agonist vilanterol, causing tachycardia, palpitations, hypertension and arrhythmias. Labeling advises extreme caution during use or within two weeks of stopping a tricyclic. Umeclidinium adds anticholinergic effects that may be additive with imipramine.
What this interaction means in plain words
Imipramine and Umeclidinium bromide / vilanterol inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Tricyclics block norepinephrine reuptake and may potentiate the vascular and cardiac effects of beta-agonists, with additive anticholinergic activity from umeclidinium.
What to do
Monitor heart rate, blood pressure and ECG if needed, and watch for palpitations or arrhythmia. Consider an alternative bronchodilator if cardiovascular effects emerge.
