Summary

Both drugs increase water and sodium loss, and together may cause excessive volume depletion, hypotension and rapid changes in serum sodium. Overly fast sodium correction carries a risk of osmotic demyelination, and indapamide itself can cause hyponatremia.

What this interaction means in plain words

Indapamide and Tolvaptan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Indapamide belongs to Cardiovascular; Tolvaptan belongs to Endocrine.

Evidence level: Moderate

Why it happens

Additive diuretic effects: tolvaptan causes aquaresis through V2 receptor blockade while indapamide causes natriuresis.

What to do

Monitor serum sodium frequently, particularly during the first 24 to 48 hours of tolvaptan, and watch volume status and blood pressure. Consider holding indapamide during initiation.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
