Summary
Both lithium and volatile anesthetics such as isoflurane can prolong the QT interval, so combined use during anesthesia may add to arrhythmia risk. Lithium may also lower anesthetic requirements and potentiate neuromuscular blockade, and electrolyte shifts during surgery can heighten toxicity.
What this interaction means in plain words
Isoflurane and Lithium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Isoflurane belongs to Central Nervous System; Lithium belongs to Central Nervous System.
Why it happens
Additive pharmacodynamic QT prolongation, with lithium also affecting cardiac conduction and CNS sensitivity to anesthetics.
What to do
Obtain a baseline ECG and serum lithium and electrolyte levels before elective surgery, and monitor the ECG intraoperatively. Keep potassium and magnesium within normal ranges and consider holding lithium per perioperative protocol.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

Frequently asked questions
Can I take Isoflurane with Lithium?
MediHelp rates the interaction as moderate. Isoflurane and Lithium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
How serious is the interaction between Isoflurane and Lithium?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Obtain a baseline ECG and serum lithium and electrolyte levels before elective surgery, and monitor the ECG intraoperatively. Keep potassium and magnesium within normal ranges and consider holding lithium per perioperative protocol.
What other medicines interact with Lithium?
MediHelp also lists interactions of Lithium with, for example, Thioridazine, Vandetanib, Aceclofenac. The full list is on the Lithium page.
