Summary

Procainamide prolongs the QT interval and can cause torsades de pointes, and it also slows conduction and can cause hypotension. Isoflurane adds QT prolongation, vasodilation and myocardial depression, which together increase the risk of serious arrhythmia and hemodynamic instability.

What this interaction means in plain words

Isoflurane and Procainamide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Additive QTc prolongation and conduction slowing with additive hypotensive and negative inotropic effects.

What to do

Monitor the ECG and arterial blood pressure continuously, and correct potassium and magnesium beforehand. Use the lowest effective isoflurane concentration and be ready to treat hypotension and arrhythmias.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
