Summary

Aminoglycosides such as tobramycin can impair neuromuscular transmission and may amplify the effect of botulinum toxins. This can produce excessive muscle weakness, including swallowing and breathing difficulty, particularly with systemic aminoglycoside use.

What this interaction means in plain words

Letibotulinumtoxina and Tobramycin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive pharmacodynamic impairment of neuromuscular transmission, with aminoglycosides reducing presynaptic acetylcholine release on top of toxin-mediated blockade.

What to do

Avoid starting systemic tobramycin around the time of botulinum toxin treatment when alternatives exist. If both are required, monitor for excessive weakness, dysphagia and respiratory compromise.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
