Summary
Barbiturates induce hepatic enzymes and may increase lidocaine clearance, lowering its effect. The magnitude is variable and more relevant with chronic barbiturate use and systemic lidocaine. Additive CNS depression is also possible.
What this interaction means in plain words
Lidocaine and Pentobarbital can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Barbiturate induction of hepatic CYP enzymes (including CYP3A4 and CYP1A2) increases lidocaine metabolism.
What to do
Monitor for reduced antiarrhythmic or anesthetic effect and titrate lidocaine to response, using concentrations if available. Watch for additive sedation.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
