Summary
Systemic lidocaine, such as intravenous use as an antiarrhythmic, may add to the QT-prolonging and proarrhythmic potential of pimozide. Pimozide is a high-risk QT-prolonging drug and torsades de pointes is a concern. Local anesthetic use gives minimal systemic exposure and is less relevant.
What this interaction means in plain words
Lidocaine and Pimozide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Lidocaine belongs to Cardiovascular; Pimozide belongs to Central Nervous System.
Why it happens
Additive cardiac electrophysiologic effects with pimozide-related QT prolongation.
What to do
Avoid systemic lidocaine with pimozide if possible, or monitor ECG continuously and keep potassium and magnesium normal. Prefer minimal doses for local anesthesia.
