Summary

Both are class I antiarrhythmics that depress cardiac conduction, so combined use can cause additive effects such as conduction delay, hypotension and proarrhythmia. Additive CNS toxicity, such as confusion or seizures, has also been described with both agents.

What this interaction means in plain words

Lidocaine and Procainamide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Additive sodium channel blockade slowing cardiac conduction, with overlapping CNS toxicity.

What to do

Use together only with continuous ECG and blood pressure monitoring, and avoid unless clearly necessary. Reduce doses and watch for QRS widening, hypotension and neurologic symptoms.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
