Summary
ACE inhibitors can improve insulin sensitivity and may lower blood glucose in people taking antidiabetic drugs, so hypoglycemia is occasionally seen. Any ACE inhibitor-related fall in renal function can also reduce metformin clearance and raise the risk of lactic acidosis.
What this interaction means in plain words
Lisinopril and Metformin-sitagliptin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Lisinopril belongs to Cardiovascular; Metformin-sitagliptin belongs to Endocrine.
Why it happens
Additive glucose lowering through enhanced insulin sensitivity, plus possible reduced renal metformin elimination if renal function declines.
What to do
Monitor blood glucose and renal function (eGFR) after starting or changing the ACE inhibitor dose. Reduce or stop metformin-sitagliptin if renal function deteriorates significantly.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
