Summary
Parecoxib, converted to the COX-2 inhibitor valdecoxib, can blunt the antihypertensive effect of lisinopril. Combined use also raises the risk of acute kidney injury and hyperkalemia, especially with dehydration, in older adults or with existing renal impairment.
What this interaction means in plain words
Lisinopril and Parecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
COX-2 inhibition reduces renal prostaglandin synthesis, decreasing vasodilation and sodium excretion and impairing glomerular perfusion.
What to do
Ensure adequate hydration, use the shortest course possible, and monitor blood pressure, creatinine and potassium.
