Summary
ACE inhibitors reduce aldosterone and potassium excretion, and potassium phosphate adds a potassium load, so hyperkalemia can develop. This is more likely with renal impairment, diabetes or rapid intravenous administration, and can cause dangerous arrhythmias.
What this interaction means in plain words
Lisinopril and Potassium phosphates can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive increase in serum potassium through reduced renal potassium excretion from ACE inhibition plus exogenous potassium.
What to do
Monitor serum potassium and renal function closely and give IV potassium phosphate at the lowest effective dose and rate. Consider holding the ACE inhibitor or using sodium phosphate if appropriate.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
