Summary

Salicylates at analgesic or anti-inflammatory doses can blunt the blood pressure lowering effect of lisinopril. Together they may also raise the risk of reduced renal function, especially in older patients, those who are volume depleted, or those with existing kidney disease.

What this interaction means in plain words

Lisinopril and Salicylates can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Inhibition of prostaglandin synthesis causes sodium and fluid retention and reduced renal vasodilation, opposing ACE inhibitor effects.

What to do

Monitor blood pressure and serum creatinine and potassium when salicylates are used regularly at higher doses, and keep patients hydrated. Prefer paracetamol for analgesia where suitable.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
