Summary
Salsalate is a salicylate NSAID that can blunt the blood pressure lowering effect of lisinopril. In patients who are volume depleted, elderly or have reduced kidney function, the combination can also cause a further decline in renal function, usually reversible.
What this interaction means in plain words
Lisinopril and Salsalate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of vasodilatory renal prostaglandin synthesis reduces renal perfusion and counteracts ACE inhibitor effects on blood pressure and glomerular hemodynamics.
What to do
Monitor blood pressure, serum creatinine and potassium after starting or changing salsalate, and ensure adequate hydration. Consider a non-NSAID analgesic such as paracetamol in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
