Summary
ACE inhibitors can raise serum lithium concentrations and cause lithium toxicity, including tremor, confusion, ataxia and renal effects. Onset may be delayed by weeks and the risk is higher with dehydration, renal impairment or older age.
What this interaction means in plain words
Lithium and Moexipril can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Lithium belongs to Central Nervous System; Moexipril belongs to Cardiovascular.
Why it happens
Reduced renal lithium clearance due to sodium depletion and altered renal hemodynamics, increasing proximal tubular lithium reabsorption.
What to do
Avoid if possible; otherwise measure lithium levels before and within the first weeks after starting moexipril and after dose changes. Reduce the lithium dose as needed and counsel on toxicity symptoms.
What to ask your pharmacist
