Summary
Tricyclic antidepressants can potentiate the cardiovascular effects of sympathomimetic agents. Salmeterol is a long-acting beta-agonist, and combined use may increase heart rate, arrhythmia risk and QT prolongation. The risk is greater in patients with heart disease or low potassium.
What this interaction means in plain words
Lofepramine and Salmeterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive adrenergic and QT-prolonging effects; tricyclics may enhance the vascular and cardiac responses to beta-agonists.
What to do
Monitor heart rate, rhythm and potassium, and consider an ECG in patients with cardiac risk factors. Use the lowest effective salmeterol dose.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
