What this interaction means in plain words

Lofexidine and Pimozide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Lofexidine belongs to Central Nervous System; Pimozide belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive QT prolongation (pimozide via hERG blockade, lofexidine via its own QT effect and bradycardia) plus additive CNS depression.

What to do

Avoid the combination if possible; if used, obtain a baseline and follow-up ECG and correct potassium and magnesium. Monitor heart rate, blood pressure and sedation.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
