Summary

Protriptyline is a tricyclic antidepressant with less sedation than others but still some CNS effects, and it can additionally blunt the antihypertensive action of centrally acting alpha-2 agonists. It also has QT-prolonging potential, which matters since lofexidine prolongs QT. The combination may reduce lofexidine's effect and add to cardiac and CNS risk.

What this interaction means in plain words

Lofexidine and Protriptyline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Tricyclics may antagonize alpha-2 agonist effects by inhibiting noradrenaline reuptake, plus additive QT and CNS effects.

What to do

Monitor withdrawal symptom control, blood pressure and ECG. Consider an alternative antidepressant if lofexidine efficacy is lost or QTc lengthens.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
