Summary
Lofexidine lowers blood pressure and heart rate, and terazosin adds vasodilation, so together they can cause marked hypotension, bradycardia and syncope. The risk is greatest during opioid withdrawal treatment when patients may also be dehydrated.
What this interaction means in plain words
Lofexidine and Terazosin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Lofexidine belongs to Central Nervous System; Terazosin belongs to Genitourinary.
Why it happens
Additive pharmacodynamic hypotension from central alpha-2 agonism plus peripheral alpha-1 blockade.
What to do
Monitor blood pressure and pulse before each dose and advise slow postural changes. Consider reducing or holding terazosin, or adjusting lofexidine, if symptomatic hypotension or bradycardia develops.
