Summary
Nafcillin can induce CYP3A4 and lower lorlatinib exposure. Lorlatinib labeling contraindicates strong CYP3A inducers because of loss of efficacy and hepatotoxicity seen in healthy volunteers with rifampin. Nafcillin is a weaker or moderate inducer, but the loss of efficacy risk remains.
What this interaction means in plain words
Lorlatinib and Nafcillin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Induction of CYP3A4 increases the metabolism of lorlatinib, reducing its plasma concentrations.
What to do
Prefer a non-inducing antibiotic alternative. If nafcillin is essential, monitor liver enzymes and the tumour response closely.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
