Summary
Barbiturates are potent inducers of CYP3A4 and can markedly lower lorlatinib concentrations, risking loss of tumour control. The strong inducer rifampin was also associated with liver enzyme elevations when given with lorlatinib. Labeling contraindicates strong CYP3A inducers.
What this interaction means in plain words
Lorlatinib and Pentobarbital are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Induction of CYP3A4 accelerates lorlatinib metabolism and lowers exposure.
What to do
Do not combine; choose a non-inducing sedative or anticonvulsant. If an inducer was used, stop it for about 3 half-lives before starting lorlatinib.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
