Summary

Antipsychotics can enhance serotonergic toxicity, and the combination of dopamine blockade with a dopaminergic MAO-B inhibitor may be counterproductive. Loxapine also has the potential to worsen parkinsonian symptoms. Rare serotonin syndrome or neuroleptic malignant syndrome has been described with similar combinations.

What this interaction means in plain words

Loxapine inhaled and Selegiline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Opposing dopaminergic effects (D2 blockade versus MAO-B inhibition), with possible additive serotonergic effects.

What to do

Monitor motor function, mental status and temperature. Seek an alternative with less D2 blockade in Parkinson disease.
