Summary

Aminoglycosides can cause renal tubular magnesium wasting, leading to hypomagnesemia during prolonged therapy. Low magnesium may cause muscle weakness, arrhythmias and secondary hypokalemia or hypocalcemia. Magnesium given as a supplement may offset this loss.

What this interaction means in plain words

Magnesium chloride and Streptomycin have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Moderate

Why it happens

Aminoglycoside-induced renal tubular injury increases urinary magnesium excretion.

What to do

Monitor serum magnesium, potassium and calcium during prolonged streptomycin courses and replace magnesium as needed.
