Summary

Oral magnesium supplements are poorly absorbed at usual doses and rarely raise serum magnesium enough to affect neuromuscular blockade. Significant potentiation of rocuronium would mainly occur with hypermagnesemia, for example in renal impairment or high doses.

What this interaction means in plain words

Magnesium supplement and Rocuronium have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Elevated serum magnesium reduces acetylcholine release and could add to rocuronium blockade, but this is only likely with hypermagnesemia.

What to do

Check serum magnesium in patients with renal impairment or high-dose supplementation before surgery, and titrate rocuronium with train-of-four monitoring.
