Summary

Mavacamten reduces cardiac contractility, and beta-blockers also have negative inotropic effects. Together they can worsen systolic function and cause heart failure, although beta-blockers are commonly used in obstructive hypertrophic cardiomyopathy. Propranolol may also affect mavacamten exposure through CYP inhibition.

What this interaction means in plain words

Mavacamten and Propranolol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Mavacamten belongs to Cardiovascular; Propranolol belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive negative inotropic effects.

What to do

Monitor left ventricular ejection fraction and clinical signs of heart failure per mavacamten labeling. Interrupt mavacamten if systolic function falls.
