Summary

Apalutamide can substantially reduce mavacamten exposure, which may result in loss of therapeutic effect on left ventricular outflow obstruction. Stopping the inducer later can also cause exposure to rise unexpectedly.

What this interaction means in plain words

Apalutamide and Mavacamten are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Apalutamide belongs to Endocrine; Mavacamten belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Apalutamide induces CYP2C19 and CYP3A4, the main enzymes that metabolize mavacamten.

What to do

Avoid the combination; use an alternative to apalutamide. Monitor echocardiographic response if the inducer is started or stopped.

What to ask your pharmacist

  • If both are essential, who is monitoring me and how often?
