Summary

Mycophenolate suppresses lymphocyte proliferation, which can allow the live attenuated vaccine viruses to cause disease and reduces the protective antibody response. Live vaccines are generally avoided in patients taking it.

What this interaction means in plain words

Measles mumps and rubella vaccine and Mycophenolate are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

Inhibition of inosine monophosphate dehydrogenase depletes proliferating T and B lymphocytes, impairing control of live vaccine viruses.

What to do

Avoid live MMR vaccine during mycophenolate treatment; give it, if needed, well before starting therapy or after immunosuppression is discontinued and immune recovery is documented.
