Summary

Measles vaccine is a live attenuated vaccine, and immunosuppressive corticosteroid therapy can allow uncontrolled vaccine-virus replication and also blunt the immune response. Disseminated vaccine-strain disease is the main concern.

What this interaction means in plain words

Measles vaccine and Triamcinolone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Corticosteroid immunosuppression impairs control of replicating attenuated virus and reduces vaccine immunogenicity.

What to do

Avoid live vaccine in patients on immunosuppressive systemic corticosteroid doses; defer until therapy has been stopped for the interval stated in labeling or guidelines (commonly at least one month). Low-dose, short-course or purely local use is usually acceptable.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
