Summary

NSAIDs such as meclofenamate can blunt the blood pressure lowering effect of sacubitril-valsartan and reduce renal function, especially in older, volume-depleted or renally impaired patients. The combination also raises the risk of hyperkalemia and acute kidney injury.

What this interaction means in plain words

Meclofenamate and Sacubitril-valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

NSAID inhibition of renal prostaglandin synthesis reduces renal blood flow and sodium excretion, which opposes the vasodilatory effect and worsens angiotensin receptor blockade-related renal hemodynamic effects.

What to do

Avoid regular NSAID use where possible and prefer paracetamol for analgesia. If the combination is necessary, monitor blood pressure, serum creatinine and potassium, and ensure adequate hydration.

What to ask your pharmacist
