Summary

NSAIDs can blunt the antihypertensive and diuretic effects of thiazides by reducing renal prostaglandin synthesis and causing sodium and fluid retention. Together with a thiazide, an NSAID can also raise the risk of acute kidney injury, especially in dehydrated or older patients.

What this interaction means in plain words

Meloxicam and Methyclothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

NSAID inhibition of renal prostaglandins reduces natriuresis and renal perfusion, antagonising thiazide effects.

What to do

Monitor blood pressure, renal function and electrolytes, and ensure adequate hydration. Use the lowest effective meloxicam dose for the shortest duration.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
