Summary
NSAIDs can blunt the blood pressure lowering effect of ACE inhibitors and can reduce renal function when combined with them. The risk is greatest in older patients, in volume depletion and in those with existing renal impairment, and acute kidney injury and hyperkalemia may occur.
What this interaction means in plain words
Meloxicam and Quinapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
NSAIDs inhibit synthesis of vasodilatory renal prostaglandins, reducing renal perfusion and opposing ACE inhibitor effects on blood pressure and glomerular filtration.
What to do
Monitor blood pressure, serum creatinine and potassium after starting or changing the NSAID, and keep the patient well hydrated. Use the lowest effective NSAID dose for the shortest time, or choose paracetamol where suitable.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
