Summary

Melphalan causes marked myelosuppression and immunosuppression, so a live attenuated varicella vaccine may replicate unchecked and cause disseminated vaccine-strain disease. The vaccine response is also likely to be blunted.

What this interaction means in plain words

Melphalan and Varicella virus vaccine live are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: High

Why it happens

Cytotoxic immunosuppression impairs control of live vaccine virus replication and reduces the immune response.

What to do

Do not give live varicella vaccine during melphalan therapy; defer it until immune recovery, generally at least 3 months after chemotherapy ends. Consider inactivated alternatives where available.
