Summary

In patients immunosuppressed by melphalan, live zoster vaccine can cause disseminated vaccine-strain varicella-zoster infection. The vaccine may also fail to produce protection.

What this interaction means in plain words

Melphalan and Zoster vaccine live are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: High

Why it happens

Cytotoxic immunosuppression impairs control of live vaccine virus replication and reduces the immune response.

What to do

Do not use live zoster vaccine during melphalan therapy; wait until immune recovery, generally at least 3 months after treatment. Use the recombinant non-live zoster vaccine if vaccination is needed.
