Methadone may add to antipsychotic-related QT effects and sedation, and both have been linked with serotonergic or dopaminergic toxicity in rare cases. The risk of serious serotonin syndrome or neuroleptic malignant syndrome is low but possible. Additive CNS depression may also occur.
What this interaction means in plain words
Methadone and Molindone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive pharmacodynamic effects on CNS depression and cardiac repolarization, with possible serotonergic and dopaminergic overlap.
What to do
Monitor for sedation, mental status change, rigidity and hyperthermia, and obtain an ECG if other QT risk factors are present. Correct electrolyte abnormalities.
