Summary

Tolmetin, like other NSAIDs, can increase and prolong methotrexate levels, particularly with high-dose methotrexate or reduced kidney function. Severe myelosuppression, mucosal injury and renal toxicity have been reported with this class.

What this interaction means in plain words

Methotrexate and Tolmetin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Methotrexate belongs to Oncology; Tolmetin belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

NSAIDs reduce renal perfusion through prostaglandin inhibition and compete for tubular secretion, lowering methotrexate clearance.

What to do

Avoid with high-dose methotrexate; with low-dose regimens monitor blood counts and renal function, or use an alternative analgesic.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
