Summary

Thiazide diuretics lower serum potassium while sacubitril-valsartan tends to raise it, so potassium effects partly offset each other. Additive blood pressure lowering and volume depletion can occur, which may impair renal function or cause hypotension.

What this interaction means in plain words

Methyclothiazide and Sacubitril-valsartan have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Moderate

Why it happens

Thiazide-induced renal potassium loss counters the reduced aldosterone-mediated potassium excretion from angiotensin receptor blockade; both also contribute additive hypotension and volume depletion.

What to do

The combination is commonly used; monitor blood pressure, serum potassium, sodium and creatinine after initiation or dose changes.
